Provost Jordan,

1.  Issue:  Student Opinion Surveys (Participation Rates): Proposed Development of Language for Inclusion on All Syllabi
In the context of the Student Opinion Survey Participation Rates, the committee recommended the development of language to inform and encourage student participation on Student Opinion Surveys for inclusion on all course syllabi.

2.  History   As you likely recall, a brief history of this matter is
a. 3 Dec 2010:  The committee approves an advisory motion referenced as FAPC Motion 2 (3 Dec 2010) To recommend to the Provost that standard language be developed that will appear on all course syllabi to inform and encourage students to participate in completing Student Opinion Surveys.
b. 3 Dec 2010The Provost is present at this meeting to receive the motion
c. 14 Jan 2011: The Provost provides an update indicating the intention to form a "writing group" to develop draft language for FAPC review.
d. 7 Feb 2011:  Draft one of this language is sent to the FAPC chair for review by FAPC
e. 4 Mar 2011:  FAPC reviews draft one and offers suggested modifications
f.  8 Mar 2011:  The writing group sends draft two, informed by 4 Mar 2011 FAPC feedback, to FAPC chair for FAPC review

3. Committee Action at 4 April 2011 FAPC Meeting:
The committee made a single modification to draft two [changing the word "virtual" to technological] and recommended draft two (as  amended) be the "standard language to appear on all course syllabi" as recommended in FAPC Motion 2 (3 Dec 2010)

DRAFT two of syllabi proposed language as amended: Given the technological sophistication of Georgia College students, the student opinion survey is being delivered through an online process. Your constructive feedback plays an indispensable role in shaping quality education at Georgia College. All responses are completely confidential and your name is not stored with your responses in any way. In addition, instructors will not see any results of the opinion survey until after final grades are submitted to the University. An invitation to complete the online opinion survey is distributed to students near the end of the semester. Your participation in this very important process is greatly appreciated.
