Dear Senate Retreat Participants,

It has been brought to my attention that we are all required to submit a “Travel Request and Authorization Form” for our Senate Retreat. I have attached a partially completed form for you to use. If you have not already submitted a travel authorization, please complete the identification section (ie name, department etc) and submit to your supervisor. They should then pass it on to the budget office as is the normal process for this form.
